This one, for reasons unknown, is copied on a Midland Valley Railroad form.
It does not seem to be a re-creation. I used to have several such examples.
I can only theorize they ran out of L&NE forms, and somehow acquired pads of
these as a stopgap for use until receiving more home road issues.
